Vision

The Executive Committee and Stakeholder Board have agreed the following vision for palliative and end of life care in Barnet, which will underpin the framework and continue to act as an inspiration to drive the project forward in its implementation phase.

It is the vision of the health, social care and voluntary sector services that every person in need of palliative or end of life care in Barnet will:

  • Have real choice in the place of their care, and the way in which their care is managed, during their last months, weeks, and days of life;
  • Have the information that they and their carers need to understand their condition and the choices that are available to them;
  • Receive high quality, planned, holistic, person-centred care, provided by skilled staff, who have a good understanding of palliative and end of life care;
  • Be able to access specialist palliative care services where their needs indicate that this is appropriate;
  • Have a seamless experience of care, provided by a well-coordinated, well-informed, multi-disciplinary team of professionals, in whom patients and their carers have confidence; and
  • Have timely access to the care and advice that they or their carers need, from needs-led, responsive and flexible services, at any time of the day or night.

All informal carers of those in need of palliative or end of life care in Barnet will:

  • Have access to a full assessment of their needs; and
  • Be able to access a range of support services to enable them to continue caring for their loved ones.
