The Lincolnshire project
The Marie Curie Delivering Choice Programme’s first flagship project was launched in Lincolnshire in October 2004. The project is now well into Phase III with service improvements being piloted across the county, delivering more choice to more patients.
Lincolnshire is the fourth largest county in England, and one of the most sparsely populated. The county has a population of over 700,000 primarily located in rural areas, with Lincoln and Boston, its two largest conurbations, accounting for only 120,000 people.
A poor road infrastructure makes travel around the county and access to services difficult, particularly in the east where there is a significant elderly need to palliative care services. Deprivation in both urban and rural areas of the county also intensifies the need for palliative care services.
By building up a full understanding of best practices among current palliative care providers and integrating new ideas from all stakeholder organisations, a number of initiatives have been developed and implemented in Lincolnshire.

Palliative Care Rapid Response Team This is a community-based nursing service which provides a resource for palliative care patients and professionals during the twilight and out of hours period.

Palliative Care Coordination Centre The Palliative Care Coordination Centre is an administrative centre that coordinates the booking of health and social home care for palliative patients and, in the future will provide a central point of communication for education services, as well as information to patients and carers.

Discharge Community Link Nurse initiative This service facilitates hospital discharge, provides advice and support to patients and carers while in hospital, provides a communication link between primary and secondary care providers and, when necessary, the nurses accompany and settle patients at their home thus ensuring continuity of care.

Education The aim of the Education and Training work stream is to ensure that health care professionals have awareness and access to recommended Palliative and End of Life Care education appropriate to their area of practice, ensuring high quality and appropriate delivery of care.

|
Support to patients and carers The objective is to provide emotional support and respite for the carers away from the patients’ home as well as training for carers in the home environment.
